TEACHERS, PLEASE COMPLETE THIS FORM to allow us to place orders for student shirts and to arrange your meals and hotel assignments. If you are attending with multiple students, please complete the form for each student. You must upload your student's revised research paper below to complete your submission.

The World Food Prize Foundation requires that Global Youth Institute student and teacher participants arrive in Des Moines for hotel check-in by Wednesday evening, October 12, in order to  take part in all three days of planned activities October 13-15. GYI Teacher/Chaperone Activity Preference Friday Afternoon (Oct. 14)

Below, please rank your Friday afternoon activity preference from 1 (first preference) through 3 (last preference).  Each teacher/adult chaperone will participate in one activity on the afternoon of Friday, October 14. We can accommodate numerous teachers in the professional development session.  However, due to the large number of total participants, we are unable to guarantee first choice placements for all teachers selecting one of the two field experiences.  We will do our best!  This is the first year that we are providing three activity options for teachers.
